Preventive Care Benefits (Preventive Care Management Program – PCMP)

Overview

The Preventive Care Benefits, provided through the Preventive Care Management Program (PCMP), are designed to keep you and your family healthier, longer—without adding extra costs to your budget.

This program gives you access to the following!

  1. No-cost preventive screenings and immunizations through in-network MagnaCare providers

  2. Women’s specialized preventive health services

  3. Telemedicine with $0 copays

  4. Mental health support and an Employee Assistance Program (EAP)

  5. Health coaching and wellness tools

  6. Prescription drug coverage

  7. A tax-free FSA card for eligible health and wellness purchases

  8. A robust digital experience through the Ignite Health App and Wellness Hub

Please Note:

The program works alongside your existing health insurance, not instead of it, to give you easier access to care, better tools to stay healthy, and meaningful tax savings, all with no change to your net take-home pay.


Who Is Eligible

You are eligible for the Preventive Care Management Program and its benefits if:

You must be an active Caresphere employee

You must have sufficient tax savings to cover the program cost

Your eligibility is based on your income, filing status, and number of dependents

You are currently employed by Caresphere and receive a regular paycheck through the company’s payroll system.

Your participation depends on whether your projected tax savings are enough to fully offset the cost of the program.

The calculation considers how much you earn along with your federal filing status and the dependents you claim

Dependents:

  • Eligible dependents typically include:

    • Your spouse

    • Your children under age 26

  • Dependents can be added for certain benefits, including:

    • Telemedicine

    • Prescription drug coverage

  • Some plans may require you to certify dependent eligibility under penalty of perjury.

What the Benefit Includes

The Preventive Care Management Program includes a wide range of services and tools. Preventive Care Benefits are at the core, supported by several additional wellness resources.A MagnaCare health insurance card displaying group name, plan details, and member ID information.

  1. Preventive Health Screenings and Immunizations

    We provide comprehensive preventive services at 100% when using in-network MagnaCare providers in NYS, including specialized care through Ignite Wellness. If you are outside of NY, it is through a PHCS network. These include:

    • Cardiovascular health screenings (blood pressure, cholesterol, aspirin use, statins)

    • Cancer screenings (colorectal, lung, abdominal aortic aneurysm

    • Mental health screenings (depression)

    • Metabolic and lifestyle screenings (diabetes, obesity, diet counseling, falls prevention)

    • Infectious disease screenings and prevention (HIV, PrEP, Hepatitis B & C, STIs, syphilis, tuberculosis)

    • Substance use screening and counseling (alcohol and tobacco cessation)

    • Recommended adult immunizations, including flu, shingles, HPV, hepatitis, tetanus, and more.

    • Routine well-baby and well-child visits; developmental, behavioral, vision, hearing, blood pressure, growth (height, weight, BMI)

    • Mental health screenings; newborn screenings (blood, bilirubin, hearing, PKU, sickle cell, hypothyroidism)

    • Immunizations from birth through age 18; oral health assessments and fluoride treatments

    • Screenings for obesity, cholesterol disorders, lead exposure, tuberculosis, and infectious diseases (HIV, Hepatitis B, STIs)

    • Substance use assessments; and preventive care and counseling for adolescents at higher risk, including PrEP when eligible.

How to Access the MagnaCare Benefit:

  • Your  MagnaCare or PHCS  card will be available to you digitally.

  • Please show this digital card at all preventive care appointments to ensure proper coverage of these essential services.

Women’s Health  Benefit
We provide comprehensive preventive services at 100% when using in-network MagnaCare providers in NYS, including specialized care tailored to women’s unique health needs through Ignite Wellness. If you are outside of NY, it is through a PHCS network. Women have access to additional specialized preventive services, ensuring they receive care tailored to their unique health needs. Highlights include:A MagnaCare health insurance card displaying group name, plan details, and member ID information.

  • Mammograms and breast cancer screenings, including BRCA risk assessments

  • Cervical cancer screening (Pap tests) and HPV testing

  • Birth control counseling and FDA-approved contraceptives

  • Counseling and support for breastfeeding mothers

  • Bone density screening for women over age 65 and for younger women who are at higher risk

  • Domestic and interpersonal violence screening and support

  • STI counseling for sexually active women

  • Tobacco use counseling and support during pregnancy

  • Gestational diabetes screening for pregnant women

  • Screenings for urinary incontinence and related infections

  • Well-woman visits that include key screenings, counseling, and forward-looking health planning

  • Rh incompatibility and preeclampsia screenings during pregnancy

  • Folic acid supplements and other prenatal preventive care, as clinically appropriate

How to Access Women’s Health Services:

  • Your  MagnaCare or PHCS  card will be available to you digitally. Please show this digital card at all preventive care appointments to ensure proper coverage of these essential services.

  1. Telemedicine
    Telemedicine access through Ignite and NYBM provides you with convenient healthcare solutions without leaving the comfort of your home. Whether you need medical advice, prescription refills, or treatment for non-emergency conditions, our telemedicine services deliver the support you need when you need it most. Our telemedicine service is suitable for a wide range of non-emergency medical concerns:

    • 24/7 access, including nights and weekends

    • Telemedicine visits are available at no cost to you

    • Comprehensive Care including:

      • Cold and flu symptoms

      • Allergies

      • Infections

      • Rashes and skin conditions

      • Annual check-ins

How to Access the Telemedicine Benefit:

  • Log in to the Ignite Health App from your device

  • Navigate to Benefits and choose the telemedicine option from the menu

  • Call (484) TELEMED at (484) 835−3633 and select the appropriate option

  • Schedule a future appointment or start an on-demand consultation

  • Connect with a healthcare professional through video or phone call

  • Discuss your symptoms and receive personalized treatment recommendations

Prescriptions can be sent directly to your preferred pharmacy if needed,making the entire healthcare process streamlined and efficient.

  1. Employee Assistance Program (EAP)
    Your Employee Assistance Program (EAP) is designed to help you reduce stress,improve mental health, and make life easier by connecting you to the right information, resources, and referrals when you need them most. All services are free, confidential, and available to both you and your family members.

    • Mental Health Sessions: Ongoing short-term mental health therapy to manage stress, anxiety, and depression, resolve conflict, improve relationships, and address any personal issues.

    • Financial consultations: Build financial wellness related to budgeting, buying a home, paying off debt, resolving general tax questions, and saving for retirement or tuition.

    • Legal referrals: Receive a free initial consultation with an attorney for personal legal matters including estate planning, wills, real estate, bankruptcy, divorce, custody, and more. *25% discount on additional services.

    • Medical advocacy: Get help navigating insurance, understanding medical bills, coverage, and claims.

    • Coaching & Wellness Support: Our Health Coaching and Wellness Support connects you with certified holistic coaches who focus on your whole well-being, from nutrition and fitness to smoking cessation.

How to Access the EAP:

  • Log in to the Ignite Health App from your device

  • Navigate to Benefits and choose the Employee Assistance Program option from the menu

  • Click either Talk to a Counselor or Talk to a Coach depending on what service you are looking for

Your sessions are confidential, as allowed by law. Your employer does not see what you discuss with your counselor or coach.

  1. Ignite Health App

    The Ignite Health App is your all-in-one hub for accessing and managing the benefits included with the Ignite Plan. This comprehensive mobile platform is designed to make health and wellness easier to navigate, more informative, and genuinely rewarding for employees.

    Within the app, you can explore your available benefits, access personalized tools, and stay informed through regular updates that ensure you’re always using the latest features and enhancements. If you have questions about your benefits or how the program works, you can get quick answers through the AI Care Navigation or connect directly with Customer Support, all without leaving the app.

    The Ignite Health App also gives you access to trusted wellness resources and partnerships, including educational health content powered by WebMD and exclusive offerings through partners like SuperPower and Henry Meds. Together, these tools help you take a more proactive, informed approach to your health, all in one easy-to-use experience.

How to Access the Ignite Health App:

  • Check your email for an invite email from Ignite Health (welcome@invites.ignitehealth.com)

  • Register for the app by clicking the Get Started button in the invite

  • Log in using the credentials from the email

  • Check out your benefits

  1. Prescription Drug Coverage
    Your Ignite Wellness prescription benefit provides exceptional coverage to ensure you and your family have access to necessary medications without financial burden.

    What’s Included:

    • $0 Complete Premium Formulary: 37 Acute, 95 ACA-MEC, and 1,000+ Chronic medications at no charge

    • A network of 70,000+ retail pharmacies across the U.S.

    • Home delivery with 90−day supplies often cheaper than 30−day retail fills

How to Access Prescription Drug Coverage

  • Open the Ignite Health app and tap the Pharmacy Program tile under My Benefits.

  • Search for your medication by name to compare prices across fulfillment options.

  • Choose how you want to receive your prescription:

    ○ Retail Pharmacy Pickup: Purchase online, receive an Rx Voucher, and pick up at a participating pharmacy with nothing owed at the counter.

    ○ Home Delivery: Place your order and receive a 90−day supply within 7−10 days. After your first order, refills are automatic.

    ○ RX Discount Card: Show your benefit card at any network pharmacy for a discounted price.

  • For the fastest home delivery, have your doctor E-scribe directly to Advanced Pharmacy, Greenville, SC (E-scribe: 4229971)

  • If your medication costs over $200, check if you qualify for the Prescription Assistance Program for additional savings.

    We support individuals and their families during challenging times by putting your extra savings through the program on an FSA Card. These FSA Cards are designed to help ease financial burdens and provide the support you need when it matters most. The FSA Card is here to enhance your overall coverage and peace of mind.

    • Use your savings on any eligible healthcare expense

    • The card gives you access to the full annual amount you had available through savings

    • Works like a debit card, allowing you to pay for eligible expenses directly at checkout (pharmacies, doctor’s offices, etc.)

Eligible Expenses Include: Copays, deductibles Prescription medications Vision and dental expenses Medical equipment Menstrual products and certain over-the-counter items

How to Access your FSA Card:

  • Your FSA card is mailed to your home address.

  • Register and activate it through the Ignite Health App or member portal.

  • Use it like a debit card to pay for eligible healthcare expenses.

  • You can also view your virtual FSA card in your Ignite Health App.

  • Please note if you do not have any leftover funds after the program cost, you will not be eligible for an FSA card.


How It Works

The Ignite Health Plan is a supplemental wellness program that works alongside your existing insurance to give you easy, no-cost access to care and health resources.

  1. Get Enrolled
    You are automatically enrolled by your employer as part of a company-sponsored wellness program—no action is required to start.

  2. Activate Your Account
    You’ll receive an email invitation to register for the Ignite Health App, where all your benefits are managed and accessed.

  3. Access Your Benefits
    Through the app, you can:

  • Connect with a doctor 24/7 via telemedicine

  • Use mental health and EAP services

  • Access prescription savings and pharmacy options

  • View preventive care benefits and resources

  1. Use Services at No Cost
    All included services are available with $0 out-of-pocket cost, as the program is funded through a pre-tax payroll structure that does not change your take-home pay.  

  2. Use Your FSA (If Eligible)
    If eligible, you will receive an FSA card to pay for approved healthcare expenses directly.

  3. Get Ongoing Support
    You can access support, resources, and care navigation anytime through the app or by contacting Ignite Health support.


Key Rules & Policies – Ignite Health Plan

The Ignite Health Plan is a supplemental wellness program (not health insurance) that provides employees with access to healthcare services at no out-of-pocket cost.

Program Structure & Payroll

  • The plan is funded through a Section 125 pre-tax structure, which reduces taxable income while maintaining the same net pay.

  • Employees will see payroll entries (pre-tax deduction, reimbursement, and tax savings), but take-home pay does not change.  

Eligibility & Participation

  • Employees are automatically enrolled by their employer but may opt out at any time by contacting support.

  • Eligible dependents (spouse and children under 26) may be added to certain benefits.  

Access & Usage Requirements

  • All services are accessed through the Ignite Health App, which is required to manage benefits, schedule telehealth visits, and access support.

  • Telemedicine is for non-emergency conditions only and is available 24/7 at no cost.  

  • Many services require use of in-network providers (MagnaCare/PHCS) for full coverage.  

Confidentiality & Privacy

  • Employee Assistance Program (EAP) services are fully confidential, and employers do not have access to personal discussions or usage details.  

FSA Card & Benefit Limitations

  • FSA card eligibility depends on available program savings; not all members will qualify.

  • Funds may only be used for eligible healthcare expenses (e.g., copays, prescriptions, medical items).  

Coverage Guidelines

  • Preventive services are covered at 100% when using in-network providers.

  • Prescription coverage includes specific formularies and programs; not all medications may be covered equally.  

Opt-Out & Support

  • Opting out results in loss of all program benefits.

General Program Questions

What is the Preventive Care Management Program (PCMP)?

The PCMP is a supplemental health and wellness program designed to provide employees with access to preventive care services such as telehealth, mental health support, prescription drug coverage, an FSA card for eligible expenses, and more—at no out-of-pocket cost to you.

Why am I enrolled in this program?

You have been enrolled by your employer as part of a company-wide wellness initiative. The goal is to enhance your access to healthcare, preventive services, and wellness tools while also providing tax savings for both you and the company.

Do I have to pay for this program?

No. There is no out-of-pocket cost to you. The program is funded through:

  • A pre-tax payroll deduction that lowers your taxable income

  • A post-tax reimbursement that offsets that deduction
    Your net take-home pay does not change.

What benefits am I receiving?

You are receiving:

  • A tax-free FSA card for wellness and eligible healthcare purchases

How is this different from my health insurance?

This is not health insurance. It is a supplemental wellness program that works alongside your existing health plan. It helps you access more services with no copays, bills, or deductibles for covered program benefits.

Can I opt out of the program?

Yes, you can opt out. However, it is strongly recommended that you take advantage of the benefits. If you still wish to opt out, send your full name, email, and employer name to support@ignitehealth.com. A support team member will remove you from the program and confirm.


Dependent & Family Questions

Can I add my spouse or child to the program?

Yes. As long as they meet the eligibility criteria, you can add your spouse and eligible children (usually under age 26) as dependents to certain benefits, such as Prescription Drug Coverage and Telemedicine. You will typically add them via the member portal.

How do I confirm if my dependent is eligible?

Eligible dependents typically include your spouse and children under 26. Some plans may require that you certify your dependents’ eligibility under penalty of perjury. If you have questions, contact support for guidance.


Opting Out & Support

I would like to understand the program more before opting out—who can I talk to?

If I opt out, will I lose anything else?

Yes. By opting out, you lose access to all benefits offered through the program, including:

  • Telehealth

  • The FSA card

  • Prescription drug coverage

  • EAP and mental health tools

  • Other wellness resources included in the PCMP


Payroll & Tax Questions

Why do I see new deductions and reimbursements on my paycheck?

You will see three line items related to the program:

  • PCMP125 – A pre-tax deduction that lowers your taxable income and creates tax savings.

  • PCMPREI – A post-tax reimbursement that offsets the pre-tax deduction.

  • PCMPSAV – This appears as a deduction, but it is not taken from your net pay. It represents a portion of the tax savings generated during that payroll run and is used to fund your comprehensive benefit package.

Will this change my take-home pay?

No. Your net take-home pay does not change.

How does this affect my tax return at the end of the year?

You may see a slightly smaller tax refund because you paid less in taxes throughout the year. You are not losing money—you simply overpaid less in taxes.

What is a Section 125 Plan, and how does it benefit me?

A Section 125 Plan allows certain benefits to be paid with pre-tax dollars. This reduces your taxable income, which lowers your federal income and FICA taxes and increases your real take-home value.

Why does my W-2 look different now?

Your W-2 may show a lower taxable wage amount because of the pre-tax deduction used to fund the program. This is expected and fully compliant with IRS regulations.



Using the Program

How do I log into the ignite Health App or member portal?

  • Look for your welcome email from Ignite Health.

  • Follow the instructions to register and create your account.

  • If you do not see the email, check your spam or promotions folder.

  • If you still can’t find it, contact support@ignitehealth.com for help.

  • Once inside the app, go to the Benefits section to see all available tiles and instructions for each benefit (telemedicine, prescriptions, EAP, FSA, etc.).

Where do I find my prescription drug benefits?

In the Ignite Health App:

  • Open the Benefits page.

  • Click on the Prescription Drug tile.

  • Follow the prompts to activate and access your pharmacy benefits.

How do I book a telehealth appointment?

  • Log in to the Ignite Health App from your device

  • Navigate to Benefits and choose the telemedicine option from the menu

  • Call (484) TELEMED at (484) 835−3633 and select the appropriate option

  • Schedule a future appointment or start an on-demand consultation

FSA Card / Benefits Access

How do I access my FSA card?

  • Your FSA card will be mailed to your home address.

  • After you receive it, register and activate it through:

    • The Ignite Health App, or

    • Your online member portal.

What can I use the FSA card for?

You can use the FSA card for 213(d)-eligible healthcare expenses, including but not limited to:

  • Over-the-counter medications

  • Feminine care products

  • Vision products (glasses, contacts, etc.)

  • First-aid supplies

  • Eligible fitness and wellness gear

  • Mental health tools and other qualified medical expenses

When will I receive my FSA card in the mail?

Typically, your FSA card arrives within 3–4 weeks of your enrollment. If you have not received it after that timeframe.

Can I add dependents to my benefits?

Yes. You can add eligible dependents to:

  • Telemedicine

  • Prescription drug coverage
    Follow the steps in your member portal or contact support if you need help.

Can I use my FSA card at CVS and Walgreens?

Yes. Most major retailers and pharmacies, including CVS and Walgreens, accept the card. Just make sure the items you purchase are eligible healthcare expenses under Section 213(d).
