Overview

The Preventive Care Benefits, provided through the Preventive Care Management Program (PCMP), are designed to keep you and your family healthier, longer—without adding extra costs to your budget.

This program gives you access to the following!

  1. No-cost preventive screenings and immunizations through in-network MagnaCare providers

  2. Women’s specialized preventive health services

  3. Telemedicine with $0 copays

  4. Mental health support and an Employee Assistance Program (EAP)

  5. Health coaching and wellness tools

  6. Prescription drug coverage

  7. A tax-free FSA card for eligible health and wellness purchases

  8. A robust digital experience through the TEG Health App and Wellness Hub

Please Note:

The program works alongside your existing health insurance, not instead of it, to give you easier access to care, better tools to stay healthy, and meaningful tax savings, all with no change to your net take-home pay.


Who Is Eligible

You are eligible for the Preventive Care Management Program and its benefits if:

You must be an active Caresphere employee

You must have sufficient tax savings to cover the program cost

Your eligibility is based on your income, filing status, and number of dependents

You are currently employed by Caresphere and receive a regular paycheck through the company’s payroll system.

Your participation depends on whether your projected tax savings are enough to fully offset the cost of the program.

The calculation considers how much you earn along with your federal filing status and the dependents you claim

Dependents:

  • Eligible dependents typically include:

    • Your spouse

    • Your children under age 26

  • Dependents can be added for certain benefits, including:

    • Telemedicine

    • Prescription drug coverage

  • Some plans may require you to certify dependent eligibility under penalty of perjury.

What the Benefit Includes

  1. Preventive Care through MagnaCare

    When using in-network MagnaCare providers, adults enrolled in this plan have access to comprehensive preventive services covered at 100%. These include:

    • Annual screenings for Blood pressure, Cholesterol, Type 2 diabetes.

    • Cancer screenings: Colorectal cancer, Lung cancer (for eligible current or former smokers), Hepatitis screenings

    • Immunizations: Flu, Hepatitis A and B, HPV, MMR (measles, mumps, rubella), Shingles, Tetanus and others as recommended

    • HIV screening and PrEP medication for eligible adults

    • STI prevention counseling and screening

    • Depression screening and counseling

    • Alcohol misuse screening and counseling

    • Obesity screening and diet counseling for chronic disease prevention

    • Tobacco use screening and counseling, including help and resources to quit

    • Screenings for conditions such as tuberculosis or syphilis in individuals at higher risk

    • Counseling to prevent falls and injury in older adults

How to Access the MagnaCare Benefit:

  • Watch for your MagnaCare packet and card (physical or digital).

  • Find an in-network provider using MagnaCare’s provider directory or your packet.

  • Schedule preventive visits (annual physicals, screenings, immunizations, etc.).

  • Show your MagnaCare card at each preventive visit to ensure coverage.

  1. Women’s Health Preventive Services
    Women have access to specialized preventive care through MagnaCare and TEG Wellness. These services are covered at 100% when using in-network providers and may include:

    Mammograms and breast cancer screenings, including BRCA risk assessments

    • Cervical cancer screening (Pap tests) and HPV testing

    • Birth control counseling and FDA-approved contraceptives

    • Counseling and support for breastfeeding mothers

    • Bone density screening for women over age 65 and for younger women who are at higher risk

    • Domestic and interpersonal violence screening and support

    • STI counseling for sexually active women

    • Tobacco use counseling and support during pregnancy

    • Gestational diabetes screening for pregnant women

    • Screenings for urinary incontinence and related infections

    • Well-woman visits that include key screenings, counseling, and forward-looking health planning

    • Rh incompatibility and preeclampsia screenings during pregnancy

    • Folic acid supplements and other prenatal preventive care, as clinically appropriate

  • Find an in-network provider using MagnaCare’s provider directory or your packet.

  • Use the TEG Health app to find providers.

  1. Telemedicine
    Telemedicine access through TEG and NYBM allows you to connect with a provider from home, with:

    • 24/7 access, including nights and weekends

    • No-cost telemedicine visits

    • Support for non-emergency conditions such as:

      • Cold and flu symptoms

      • Allergies

      • Minor infections

      • Rashes and skin conditions

      • Annual check-ins and follow-ups
        Providers can also send prescriptions to your preferred pharmacy when medically appropriate.

  • Your FSA card is mailed to your home address.

  • Use it like a debit card to pay for eligible healthcare expenses.

  • You can also view your virtual FSA card in your TEG Health App.


How It Works

  1. Enrollment

    • You are enrolled in the Preventive Care Management Program by your employer as part of a company-wide wellness initiative.

    • Enrollment activates your access to:

      • Preventive care through MagnaCare

      • Telemedicine and EAP

      • Prescription benefits

      • FSA card

      • TEG Health App and wellness resources

  2. Preventive Care Visits

    • You receive your MagnaCare card by mail or digitally.

    • You schedule preventive care appointments with in-network MagnaCare providers.

    • When the visit qualifies as preventive under the guidelines and is with an in-network provider, it is covered at 100% (no copays or out-of-pocket costs for the preventive portion).

  3. Program Funding and Payroll

    • The program is funded through a combination of:

      • A pre-tax payroll deduction

      • A post-tax reimbursement

    • Your net take-home pay remains the same.

    • Part of your tax savings help fund your enhanced benefits package and FSA card.

  4. Use of Other Benefits

    • Telemedicine: call or use the app for 24/7 virtual care (no copays).

    • EAP: log into the TEG Health App or call the EAP number for counseling, financial and legal help, and life-resource referrals.

    • Health Coaching: select a coaching benefit in the app and schedule sessions.

    • Prescription Coverage: activate via your welcome email, then use your digital pharmacy card.

    • FSA Card: use at checkout for eligible health expenses once you receive and activate it.


Important Rules & Policies

  • Preventive care services are covered at 100% only when:

    • The provider is in-network with MagnaCare, and

    • The service meets preventive care guidelines (routine screening, not diagnostic).

  • This program is supplemental and does not replace your primary health insurance.

  • Certain preventive services may depend on:

    • Age

    • Gender

    • Risk factors (such as smoking history, pregnancy, or chronic conditions)

  • Enrollment in the program includes:

    • A pre-tax deduction and a post-tax reimbursement on your paycheck

    • A separate line related to tax savings (PCMPSAV) that reflects how the program is funded but does not reduce your net pay

  • Your taxable income is reduced due to pre-tax deductions, which:

    • Lowers your taxes throughout the year

    • May result in a slightly smaller refund (because you overpaid less)

  • Opting out:

    • You may opt out of the program, but you will lose access to all PCMP benefits, including:

      • Telemedicine

      • FSA card

      • Prescription drug coverage

Yes, you can opt out. However, it is strongly recommended that you take advantage of the benefits. If you still wish to opt out, send your full name, email, and employer name to info@tegwellness.com. A support team member will remove you from the program and confirm.

Can I add my spouse or child to the program?

Yes. As long as they meet the eligibility criteria, you can add your spouse and eligible children (usually under age 26) as dependents to certain benefits, such as Prescription Drug Coverage and Telemedicine. You will typically add them via the member portal.

How do I confirm if my dependent is eligible?

Payroll & Tax Questions

Why do I see new deductions and reimbursements on my paycheck?

You will see three line items related to the program:

  • PCMP125 – A pre-tax deduction that lowers your taxable income and creates tax savings.

  • PCMPREI – A post-tax reimbursement that offsets the pre-tax deduction.

  • PCMPSAV – This appears as a deduction, but it is not taken from your net pay. It represents a portion of the tax savings generated during that payroll run and is used to fund your comprehensive benefit package.

Will this change my take-home pay?

No. Your net take-home pay does not change.

How does this affect my tax return at the end of the year?

You may see a slightly smaller tax refund because you paid less in taxes throughout the year. You are not losing money—you simply overpaid less in taxes.

What is a Section 125 Plan, and how does it benefit me?

A Section 125 Plan allows certain benefits to be paid with pre-tax dollars. This reduces your taxable income, which lowers your federal income and FICA taxes and increases your real take-home value.

Why does my W-2 look different now?

Your W-2 may show a lower taxable wage amount because of the pre-tax deduction used to fund the program. This is expected and fully compliant with IRS regulations.



Using the Program

How do I log into the TEG Health App or member portal?

  • Look for your welcome email from TEG Health.

  • Follow the instructions to register and create your account.

  • If you do not see the email, check your spam or promotions folder.

  • If you still can’t find it, contact info@tegwellness.com for help.

  • Once inside the app, go to the Benefits section to see all available tiles and instructions for each benefit (telemedicine, prescriptions, EAP, FSA, etc.).

Where do I find my prescription drug benefits?

In the TEG Health App:

  • Open the Benefits page.

  • Click on the Prescription Drug tile.

  • Follow the prompts to activate and access your pharmacy benefits.

How do I book a telehealth appointment?

  • Open the TEG Health App, select the telemedicine option, and follow the prompts to start an on-demand visit or schedule a future appointment.

FSA Card / Benefits Access

How do I access my FSA card?

  • Your FSA card will be mailed to your home address.

  • After you receive it, register and activate it through:

    • The TEG Health App, or

    • Your online member portal.

What can I use the FSA card for?

You can use the FSA card for 213(d)-eligible healthcare expenses, including but not limited to:

  • Over-the-counter medications

  • Feminine care products

  • Vision products (glasses, contacts, etc.)

  • First-aid supplies

  • Eligible fitness and wellness gear

  • Mental health tools and other qualified medical expenses

When will I receive my FSA card in the mail?

Typically, your FSA card arrives within 3–4 weeks of your enrollment. If you have not received it after that timeframe, contact (855) 374-6431 for assistance.

Can I add dependents to my benefits?

Yes. You can add eligible dependents to:

  • Telemedicine

  • Prescription drug coverage
    Follow the steps in your member portal or contact support if you need help.

Can I use my FSA card at CVS and Walgreens?

Yes. Most major retailers and pharmacies, including CVS and Walgreens, accept the card. Just make sure the items you purchase are eligible healthcare expenses under Section 213(d).
