Overview

CareSphere offers a full suite of voluntary insurance benefits through Aflac, designed to provide extra financial protection when you need it most. Aflac pays cash directly to you, not to doctors or hospitals, so you can use the money however you choose. These benefits are pre-tax and help cover unexpected costs associated with accidents, illness, hospital visits, and more.

Who Is Eligible

All active CareSphere employees are eligible to enroll in Aflac benefits. You may enroll:

During Onboarding

During the annual open enrollment period

After a qualifying life event

list of qualifying life events:

  • Marriage or domestic partnership, Divorce or legal separation, Birth or adoption of a child, Death of a dependent or spouse, Gain or loss of other coverage, Change in dependent eligibility, Change in employment status, Significant cost or coverage change, Relocation, Court order, Loss of Medicaid or CHIP eligibility, Enrollment or disenrollment in Medicare.

Please Note:

You may also elect coverage for your spouse and/or children, depending on the plan.


What the Benefit Includes

CareSphere offers the following Aflac plans:

🚘 Accident Plan

Pays cash for injury-related services such as:

  • Initial and follow-up visits

  • Hospitalization

  • Surgeries, MRIs, and X-rays

  • Crutches, stitches, and burns

  • Physical therapy

  • Annual wellness benefit ($60)

Sample payout for a broken leg (no surgery):

  • Initial Visit: $120

  • MRI: $200

  • Hospital Stay (1 day): $1,250

  • Specific Injury Amount: $625

  • Crutches: $125

  • 2 Follow-up Visits: $50

  • Total Benefit: $2,370

Weekly Pre-Tax Rates:

  • Individual: $4.47

  • Single Parent: $7.08

  • Insured & Spouse: $6.15

  • Family: $8.76

🏥 Hospital Plan

Provides cash benefits for hospital stays and ER visits. Rates vary by age and family structure. (Example rates below are weekly pre-tax deductions.)

😷 Cancer Plan

Pays a lump sum and ongoing treatment support for cancer diagnoses:

  • $1,000 on initial diagnosis

  • Chemotherapy and radiation support

  • Surgery, bone marrow/stem cell transplant

  • Transportation, lodging, home nursing

  • Egg harvesting, hospice

  • Annual wellness benefit ($25/person)

Weekly Rates (Ages 18–75):

  • Individual/Single Parent: $5.94

  • Insured & Spouse/Family: $10.41

  • Dependent Child Rider: $0.21

🤒 Critical Illness & Recovery

Cash benefits up to $20,000 for conditions like:

  • Heart attack

  • Stroke

  • Coma

  • Organ transplant

  • ICU stay

  • Coronary artery bypass

  • Paralysis

💉 Short-Term Disability (Post-Tax)

Provides income if you are unable to work due to a covered illness or injury:

  • $1,000/monthly benefit

  • Covers accidents and illnesses

Weekly Rates:

  • Age 18–49: $11.40

  • Age 50–64: $14.40

  • Age 65–74: $18.30

🧬 Life Insurance

Customized coverage designed to support your family during difficult times. Rates vary and are based on your individual profile.


How It Works

  1. Enroll during onboarding or open enrollment

  2. Choose your plan(s) based on your financial needs and personal situation

  3. Pay via payroll deductions—pre-tax or post-tax depending on the plan

  4. File a claim directly with Aflac when eligible

  5. Receive cash benefits directly to use however you need, medical bills, groceries, rent, etc.
