From core medical coverage to dental, vision, and critical illness protection, our benefit plans prioritize your wellness, flexibility, and financial security.

Employees can choose from the following benefit plans:

  • WellCare, FlexCare, KeyCare, VitalCare, MV 6500, Critical Care (Voluntary), Delta Dental, VSP Vision, ExtraCare Low

All plans are conveniently managed through the HealthWallet App, which gives you access to digital ID cards, benefit details, and provider networks at your fingertips.

All full-time CareSphere employees are eligible to enroll in health insurance plans:

During Onboarding

During the annual open enrollment period

After a qualifying life event

  • Marriage or domestic partnership, Divorce or legal separation, Birth or adoption of a child, Death of a dependent or spouse, Gain or loss of other coverage, Change in dependent eligibility, Change in employment status, Significant cost or coverage change, Relocation, Court order, Loss of Medicaid or CHIP eligibility, Enrollment or disenrollment in Medicare.

Medical Plans

WellCare

  • Designed for preventive care and basic outpatient services

  • Includes office visits, labs, x-rays, urgent care, and prescription discounts

  • Uses PHCS network for in-network services

FlexCare

  • Enhanced preventive care and outpatient services

  • Includes labs, x-rays, imaging, and surgical outpatient services

  • Some hospital indemnity benefits

KeyCare

  • Higher-level outpatient and inpatient hospital benefits

  • Includes physician services, anesthesia, diagnostic services, and urgent care

  • Additional surgical and hospitalization indemnity benefits

VitalCare

  • Comprehensive coverage for outpatient, emergency, and inpatient services

  • Includes surgical services, mental health coverage, ICU, and ER

  • Higher indemnity payments for hospital stays and emergency situations

MV 6500 (Minimum Value Plan)

  • Includes preventive care, doctor visits, prescriptions, labs, and ER

  • Highest level of indemnity support for inpatient and ICU stays

  • Offers minimum essential coverage for ACA compliance

Critical Care

  • Lump-sum payments for covered illnesses like cancer, stroke, and heart attack

  • Coverage amounts up to $15,000

  • Helps cover out-of-pocket expenses during a serious diagnosis

    CRITICAL CARE 10,000

    CRITICAL CARE 25,000

ExtraCare Low

  • Hospital confinement and surgery support

  • Provides daily benefit payments for hospital stays

  • Includes surgical indemnity, outpatient, and physician services


Dental & Vision Plans

Delta Dental

  • Annual max benefit: $1,500

  • Covers preventive, basic, and major services (cleanings, fillings, crowns)

  • Orthodontics coverage available for dependents

VSP Vision

  • One exam every 12 months

  • Coverage for lenses and frames or contact lenses

  • Access to VSP's national provider network

How It Works

  1. Choose Your Plan during onboarding or open enrollment based on your needs

  2. Enroll in medical, dental, vision, and/or voluntary benefits

  3. Download the HealthWallet App to view digital ID cards, access Recuro Health, and locate providers

  4. Visit In-Network Providers to maximize benefits and avoid out-of-network charges

  5. Use Recuro for Virtual Care: 24/7 urgent care and mental health services via app or phone

  6. Receive Services and present your insurance ID at provider visits

  7. Monitor Claims and usage via HealthWallet and provider portals

If you don’t actively enroll in a health plan during your onboarding or open enrollment period, your coverage will be automatically waived. This means you will not have insurance through CareSphere unless you experience a qualifying life event that allows you to enroll later.

How often can I use vision or dental services?

You can have an eye exam every 12 months with VSP Vision and two cleanings per year with Delta Dental.

Is MV 6500 considered major medical insurance?

Yes, MV 6500 is a Minimum Value Plan that meets ACA guidelines.

Can I use any doctor for my health insurance?

To avoid out-of-network costs, you should use PHCS providers.
