Plan Of Care (POC)

A Care Plan (A.K.A Plan of Care or POC) is your step-by-step game plan for taking care of our awesome patients. It's all about helping them out with their personal care, medical, mental health, and other needs we figured out from our assessments. 🏥🧠🏠

Now, the Care Plan isn't some boring list of rules. Nope, it's like your trusty guide for when you're at the patient's place. It tells you what needs to happen, when it needs to happen, and how often. It's got all the deets! 📅✅

Plus, we're all about making our patients happy, so the patient has any special preferences on how things should be done, we've got it covered in there too. We're all about personalized care! 🤩

So, there you have it, the Care Plan is like your secret weapon to rock the caregiving game and make a real difference in our patient's lives! 🚀🙌💖

📝 Will the CarePlan be reviewed with me before taking on a case?

Absolutely! Remember that Care Plan we just talked about in the previous section? Before agreeing to take on any case, the coordinator gives you a call and goes through the entire Care Plan to ensure you are clear of the tasks. They want to make sure you're crystal clear about what's on the agenda. ☎️✅

Then, the Care Plan gets attached to the case assignment contract which you sign when you agree to the case. It's also attached in the confirmation email after you sign the contract. So, you've got all the info right there.

But here's the deal: don't just review it once and forget about it. You gotta review it every single day before you head to your patient's place. Things might change, and you've gotta stay up to date. So, keep that Care Plan handy, and you're good to go! 🚀👍💪

📱 Having Your POC Handy:

Okay, so you want to have your Care Plan ready at your fingertips while working with your patient, right? We've got you covered!

First off, you can access it on the HHAExchange mobile app. It's there for you anytime you need it. Here's how you do it: Log into the HHAeXchange app, find the visit you're looking for, and head over to the Care Plan tab. Your POC is listed right there, easy-peasy. 📱✨

Now, here's the important stuff: If your patient or their family asks you to do something that's not in the Care Plan, pick up that phone and call the patient's coordinator. You've got to make sure it's all good and that you've got the green light for that task. Stick to what's in the Care Plan. If something's missing or needs a tweak, our Coordinator and/or Nurse can update it.

Oh, and one more thing: your main focus is the patient, alright? You're not the family's handyman. So, when it comes to light housekeeping, you take care of the patient's bedroom and the areas they use, like the bathroom and kitchen. You don't need to worry about cleaning up the other family members' spaces. 🏡🚿🍽️

If you ever need any changes, disagree with a task, or want to discuss the Care Plan, don't hesitate. Reach out to the coordinator ASAP, and we'll get things sorted out on our end. Your commitment to these details is what makes us a top-notch team! 🌟🤝💼

📱 Viewing the Plan of Care on the HHAeXchange app

• Navigate to the Patients tab.

• Click on the name of the patient.

• On the VISITS tab, select the date of which you are looking for the Care Plan. Keep in mind, a patient can have a different CarePlan for each day of the week. patients tab

• Scroll over to the Care Plan tab to view the detailed Plan of care. patients tab

⚠️ If you are unable to see the Care Plan for a client on the mobile app, you must call the patient's coordinator immediately for them to correct this

Before clocking out. You will be required to document exactly which poc task were done that day. The care plan is your guidebook for your shift duties.

Case Review

After submitting the form, a CareSphere coordinator is all set to give you a ring on the phone. Be ready to answer that call because you don't want to let that perfect case slip through your fingers to another superstar.

📞 During the phone call, our coordinator will give you the lowdown on the case details. You'll learn about the patient's age and gender, their diagnosis or condition, and the specific tasks and needs involved. Plus, you'll get the exact address, so you'll know exactly where you're headed.

💭 Now, if you've got any other burning questions or specific concerns, this is your golden moment! Don't be shy to chat with the coordinator about allergies to pets, if you're not a fan of chatty parrots, or if you can't handle the smoky jazz – this is your chance to make sure it's a perfect fit.

No problem, just be upfront with the coordinator.

Because, my friend, the next step is the big one – finalizing a contract.

📝 Once you put your signature on that dotted line, you're all in.

So, take your time, ask your questions, and ensure you're ready for the journey ahead.

Meet and Greet

🤝 When you're assigned a new case, sometimes patients want to meet you before diving into care. When you agree to a meet and greet visit, you have not yet committed to permanently taking on the case. You will have the chance to commit to the case after the meet and greet.

⏰ Remember, this time is considered work, so don't forget to clock in when you start and out when you're done. You'll get paid for the meet and greet.

Typically, a meet and greet visit will last about 30 minutes to an hour.

✨ Here's what to do during a meet and greet:

Introduction: Start by introducing yourself and sharing a bit about your background.

Put the Patient at Ease: Make them feel comfortable by asking about their interests and hobbies. It's like finding common ground to chat about.

Review the Plan: Go over the tasks outlined in the Plan of Care (POC) to see if they're comfortable with it. It's a bit like trying on a new pair of shoes to see if they fit.

Confirm Your Interest: After the meet and greet, tell the coordinator right away if you like the patient. It's a two-way street – you have to like them, and they should like you. If both parties are a match, you'll be assigned to that patient permanently.

📌 Additional Note:

If the patient doesn't request a meet and greet but you feel it's necessary, that's totally fine. Just inform the coordinator before accepting the permanent case, and they'll help set it up.

Our goal is to ensure that when you start working on the first day, and you've signed the case assignment, you're 100% ready to commit to the case.

💙 This process is all about building the right connection and making sure both you and the patient are comfortable working together.

It's a bit like dating – you want to make sure it's a match before committing to a long-term relationship.

If you need to call off, make sure to follow our call-off rules.

You'll be added to the patient's schedule, so hold onto that contract – it's your go-to source for patient information and the plan of care.

Assigned Shift Schedule

🎶🕥💪 Alright, so you've got your shift, and it's written in the contract like a sacred pact.

Now, let's dive into some key rules that'll keep your schedule as solid as a rock concert ticket.

These are the dos and don'ts when it comes to your assigned hours – let's make sure you're in perfect harmony with your caregiving schedule!

📅 Scheduled Hours:

Your work schedule is determined and set by the Coordinator.

This schedule is designed to ensure that all required care hours are met for the patient.

🔄 Schedule Changes:

If a patient or their family requests any changes to the schedule, such as different hours or shifts, you must contact the Coordinator immediately to request approval.

Changes should not be made without prior authorization.

🏠 Client Presence:

You should never be in the client's home if the client is not present.

If the client leaves their home for any reason, get in touch with the Client Service Coordinator immediately for further instructions.

Safety and security are top priorities.

Overtime Authorization:

If the patient or their family requests you to stay longer than the scheduled time, it's crucial to consult your Coordinator for approval before agreeing to extend your shift.

Unauthorized overtime cannot be reimbursed, and we all know you're here to get paid for your hard work.

Utilization of Hours:

It's your responsibility to ensure that you fulfill the weekly hours specified for the patient.

This information is available on your case assignment contract and the Perry system.

Unless you're on vacation, there should be no unaccounted for hours, and you should strive to meet the required hours every single week.
